We are here for our patients. We are working on establishing a telemedicine (video visit) option for patients. As soon as this is available we will update this page.

In the meantime, we will continue seeing patients in the office.  

Office is open for patients by appointment only.

All patients are required to be screened prior to coming.

For the Health and Safety of our patients and staff we have made the following changes to our In-Person Office Hours:

Dr. Dominick Condo M - W -Th - F 8:00am - 4:00pm

Dr. Hammad Choudhry M -T - Th 10:00am - 4:00pm

Renee Trotter M - W -Th - F 8:00am - 4:00pm

To ensure the safety and well-being of both our patients and staff:

• All staff wear PPE.

• All patients are required to wear a mask while in the office.

• Fully Vaccinated patients are asked to bring their COVID vaccine card to be added to their chart

• Office is open for patients only. If you require assistance please call the office.

• All patients are screened for symptoms prior to their appointment.

• All patient areas are cleaned/sanitized throughout the day.

• Entire office is professionally cleaned and sanitized each evening.

Plan to arrive 10-15 minutes before your scheduled appointment time. You will be asked to leave if you do not have an appointment within the next 15 minutes. We must limit the number of people in our office at any given time. This is to allow for practicing proper Social Distancing to minimize spread of infection.

Any patient that has developed or is experiencing COVID-like symptoms, or has been exposed to someone with symptoms or a confirmed COVID diagnosis, should call the office before coming.

Together, we will get through this Public Health Emergency.
