We are closed holidays including New Year's Day, Good Friday, Memorial Day, July 4th, Labor Day, Thanksgiving and the day after, Christmas Eve & Christmas

 

What is a Patient Centered Medical Home?

It is a new approach to care where all your health needs are coordinated through a care team managed by a primary care physician. This ensures you receive the necessary care when and where you need it, in a manner you can understand.

 

Why is a Patient Centered Medical Home important?

It places you at the center of the delivery of care. It works on behalf of the patient to coordinate care with the many health professionals (specialist physicians, hospitals, skilled nursing facilities, home care, imaging centers) and work in ways that are most responsive to your needs.  The goal is for the right job to be performed effectively by the right person at the right time.

 

What is shared decision making?

We create a partnership with you and your family to ensure that well-informed decisions respect your wants, needs and preferences. We will always be available to provide education and support so that you can take an active role in your care.

 

What do you mean by 24/7 access?       

You will have access to your electronic medical record via a secure online portal at any time of day, every day of the year. You will have the ability to book appointments and renew prescriptions via phone, app, or online. You can email your provider directly. Providers will have 24/7 access to your medical record should you need support by phone.

 

What are the costs of being part of a Patient Centered Medical Home?

The coordinated and personalized care you receive at a patient centered medical home like FirstCare Medical Group does not cost more. In fact, it can improve your health while saving on costs like unnecessary procedures. However, it does ask that you play an active role as part of the care team. 

 

How do I join?

Just tell us – in person, on the phone, or via secured messaging on the MyHealth Portal

 

What do I need to bring to a visit?

Your insurance card and a photo ID. Please note, you can pre-register online through our MyHealth Portal
